We initiate care, a treatment plan, and relationships
over the initial visit:

We gather demographic information

Hear and assess any urgent medical issues

Provide necessary prescriptions

NP/PA has MD Available

MD Visit may occur upon medical necessity

Subsequent visits offered as often as every week or as needed and/or requested*

Our medical provider will be present at your Assisted Living community one-two days every week. We are available by telephone  “on-call” after hours and on the weekends when your needs may arise. Laboratory tests and x-ray services can be obtained in the comfort of your home. We store our patient’s medical records electronically and have access to them while in the home.
